The Behavioral Health Consultant (BHC) provides supportive advocacy to patients and their families as part of an interdisciplinary health care team to overcome barriers to progression of care. The BHC works with chronically ill patients to assist them with management of the behaviors affecting their health and coordinates healthcare needs and wellness goals with providers, nurse care managers, case management, and other Cox Health staff.
Responsibilities- Advocate for patients and families within the healthcare team
- Assist chronically ill patients in managing health-related behaviors
- Coordinate care plans with multiple healthcare professionals
- Provide patient-centered communication and support
- Master’s Degree in Social Work, Counseling, or PsyD (required)
- Active LCSW Licensure, LPC, or Missouri State Licensed Psychologist (required)
- Experience with electronic health records or prior work in a family practice medical office (preferred)
- Excellent telephone and interpersonal communication skills
- Strong organizational skills with ability to multi-task and prioritize
- Solid knowledge of medical terminology
- Computer proficiency (required)
