Status: Non‑exempt

Working Conditions: Works in a typical office setting and from the employee’s home or other community location as established by a flexible work agreement. Performs paperwork, documentation, and other indirect services. Meets clients in homes, foster homes, residential facilities, hospitals, or other community locations as needed; drives an automobile routinely in all area types; requires reliable transportation. Requires sitting, standing, bending, reaching, and lifting up to 25 pounds.

Requires manual dexterity to operate computers, copiers, calculators, telephone, and other office equipment. Requires normal hearing and vision. Must be able to visit more than one family per day.

Position Summary

The Foster Care Case Manager (FCCM) is responsible for ensuring safety and providing services to children and families in foster care. Tasks include facilitating Family Support Team (FST) meetings, developing individualized care (treatment) plans, coordinating individualized services across all settings, managing providers in the home, school, and community, and documenting support-related activities and services. The FCCM promotes effective utilization of support services to achieve permanency for children and families.

Job Functions
  • Follow all contractual and agency policies, procedures, and seamless delivery of services to children and their families.
  • Maintain a caseload of 12 to 15 children, depending on program capacity and agency needs.
  • Provide full case management services to children and their families within the Missouri foster and adoptive care system from assignment until case closure or transfer.
  • Document all activities and maintain accurate data entry in FACES (state online database system) and CSWIS (consortium online database system).
  • Have face‑to‑face contact with each assigned child and family at least monthly (may require extensive travel); maintain telephone contact with providers regularly to review activities and progress.
  • Evaluate and factually report impact of services to team members and judiciary; attend all court hearings; schedule, coordinate, and facilitate FST meetings within prescribed time frames.
  • Participate in the FCCM consortium on‑call schedule as employee on caseload (EOC) for PCHAS on a rotating basis.
Requirements Education & Experience
  • Bachelor’s degree in social work, psychology, counseling, education, criminal justice, or a related human services field.
  • Master’s degree preferred.
  • Minimum of two (2) years of relevant case management experience.

  • Strong verbal and written communication, ability to represent PCHAS professionally.
  • Builds and maintains positive community relationships with personnel, placement and treatment providers, schools, and agency partners.
  • Builds rapport with children, parents, and families; actively manages and resolves conflicts.
