Reports to:
Director of Clinical Management (DCM)
Supervise, provide, and direct therapy rehabilitation services under the general supervision of the DCM, in accordance with company and facility/agency policies and all applicable federal, state, and local regulations.
Essential Functions- Provide treatment of residents/clients with injuries, illnesses, or disabilities using therapeutic activities to aid recovery and improve daily living skills.
- Practice under the supervision of a Physical Therapist (PT) and communicate resident/client progress timely.
- Participate in the development and evaluation of the plan of care with patients, representatives, and caregivers; provide education and counseling.
- Maintain timely, accurate documentation in accordance with facility/agency policy.
- Assist with marketing and program development as requested.
- Meet productivity goals and maintain an acceptable level of productivity to support facility/agency objectives.
- Continue professional development by attending in‑services and continuing education programs.
- Support facility/agency efforts to meet state, federal, and other certification standards.
- Plan and complete in‑services for staff education as requested by management.
- Attend all facility/agency meetings and in‑services.
- Maintain a positive, professional demeanor, manage time efficiently, provide leadership and promote teamwork.
- Adhere to all policies, procedures, codes of conduct, and regulatory requirements outlined in the National Church Residences Employee Information Guide.
- Respect resident rights, provide care with dignity, and report complaints and abuse to appropriate supervisors.
- Complete all annual education requirements (Compliance, Privacy, Security, HIPAA) in a timely manner.
- Perform other duties as assigned.
- Education:
Associate Degree in Physical Therapy from an approved curriculum. - Experience:
Two to four years preferred. - Travel:
Continuous for Home and Community Services staff; occasional for facility staff. - Communication:
Good verbal and written communication; proficient in reading, writing, and speaking English. - Computer:
Working knowledge of Windows, Microsoft Office Suite, and the internet. - Licensure:
Current state license to practice as a physical therapy assistant.
