Job Description

Registered Nurse Case Manager – Home Health (PRN). Mileage Reimbursement:
Service Area:
Jackson, Vinton, Pike, and Ross Counties.

We are currently seeking a dedicated and compassionate Registered Nurse Case Manager to join our Home Health team. The ideal candidate will embrace our mission and provide skilled nursing care while coordinating the overall care plan for Home Health patients and their families.

• Education:
  
  Graduate of an accredited nursing program with a current RN license.
• Experience:
  
  Minimum of two years of RN experience required. Previous home health experience is strongly preferred.

• Medical & Prescription coverage (PPO & HDHP with HSA options), including fertility benefits

• Retirement Plans: 401(a) and 403(b) with automatic 3% company contribution and an additional 3% match
