We are currently seeking a Registered Nurse (RN) Case Manager who is passionate about advancing better living and care for seniors. This role covers locations in Galloway, Grove City & Madison County and includes paid mileage reimbursement.

• Current and active RN license
• 1–2 years of home health experience required
• OASIS knowledge required
• Strong assessment and documentation skills
• Dedication to providing exceptional patient care

• Develop, implement, and update individualized plans of care, incorporating physician orders, medications, and input from clients, caregivers, and representatives.
• Complete comprehensive client assessments, including physical exams and health histories.
• Provide direct patient care, including preventive and rehabilitative procedures, medication administration, and skilled nursing interventions.
• Communicate with physicians and physician extenders regarding client conditions, updates, and care needs.
• Educate and mentor clients, caregivers, and representatives on disease management, end‑of‑life care, and home care needs.
• Collaborate with the interdisciplinary team to ensure coordinated care, discharge planning, and ongoing eligibility evaluations.
• Document all care accurately and in compliance with regulatory and organizational standards.
• Supervise and evaluate LPNs, LVNs, and Home Health/Hospice aides to ensure competency and adherence to care plans.
