Relief Nursery strengthens families with children through evidence-based interventions, intensive skill-building, parenting education, and connections to essential community resources. We are seeking a compassionate, organized, and collaborative Family Navigator / In-Home Safety and Reunification Services (ISRS) Provider to join our Springfield team.
In this role, you will work directly with families involved with ODHS Child Welfare, providing trauma-informed case management, parent coaching, life skills training, transportation in your personal vehicle, and resource navigation. Services include home visits, safety planning, and goal setting to support family stability, child safety, and successful reunification. This position operates under both our Family Navigator and ISRS contracts, requiring strong communication and documentation skills, cultural humility, and the ability to build trusting relationships with diverse families from multi-stressed environments.

- Assist with barrier removal, such as housing, mental health, substance use, and legal challenges.
- Ensure all client interactions, ODHS (Oregon Department of Human Services) communications, and community engagement activities are documented and uploaded to the shared drive within 72 hours of the interaction.
- Maintain organized and accurate records in compliance with internal documentation standards, ensuring timely and efficient retrieval of information.
- Transporting clients in your own personal vehicle as needed.
- Perform other duties as assigned by ODHS/and or Relief Nursery Supervisors.
- Implement ODHS safety and reunification plans to reduce resource care entry or duration
- Deliver skills training and support to enhance child safety and family stability
- Provide crisis intervention, parenting support, home management, and resource access
- Attend and participate in ODHS meetings and maintain regular communication with caseworkers.
- Ensure all client interactions, ODHS (Oregon Department of Human Services) communications, and community engagement activities are documented and uploaded to the shared drive within 72 hours of the interaction.
- Maintain organized and accurate records in compliance with internal documentation standards, ensuring timely and efficient retrieval of information.
- Offer culturally responsive change services and self-management support
- Perform other duties as assigned by ODHS/and or Relief Nursery Supervisors.
- Ability to pass a drug screen, criminal, and child welfare background check.
- Strong communication and professional writing skills
- Strength-based, culturally responsive approach to working with families
- Knowledge of community resources and providing basic parenting strategies.
- Ability to meet all documentation, reporting, and legal requirements
- Ability to work independently and within a team.
- First Aid/CPR certified (or able to obtain)
- Valid driver’s license, safe driving record, Personal Vehicle, and able to increase personal vehicle insurance to meet contract requirements.
- Bilingual in English and Spanish
- 2+ years of home visiting experience
- Familiarity with ODHS Child Welfare services
- Frequent travel across Lane County; transportation of clients required
- Exposure to highly stressed environments and challenging client behaviors
- Requires physical mobility in diverse home settings and extended desk work
