The Pacific Source PACE LLC is seeking a Licensed Dietitian (LD) to deliver nutrition interventions for older adults enrolled in the PACE program. You will be part of an interdisciplinary team that coordinates dietary services in the Day Health Center and collaborates with primary care providers to optimize quality of life.
You will assess nutrition needs, develop care plans, provide education to participants and families, and document care in the EHR in a compliant, timely manner.
