Job Summary

Under the supervision of an RN, the LPN provides direct nursing care within the scope of license, to patients as directed by physicians and other healthcare providers. Cooperative decision making is expected in the execution of duties.

Essential Functions
  • Implements and evaluates patient care under the supervision of an RN and participates in the development of a patient‑specific care plan.
  • Provides direct patient care in patient homes, administering medications and treatments as prescribed. Applies knowledge in edema/lymphedema management, venipuncture, management of PICC lines, implanted ports, post‑surgical drains, and pulmonary and abdominal drains.
  • Provides focused systems and wound assessment. Reports acute medical problems and condition changes to the case manager and/or attending physician, as directed.
  • Demonstrates proficiency in communicating effectively with individuals diagnosed with mental and behavioral disorders and operates efficiently in an independent home setting.
  • Documents patient care in the health record according to department standards.
  • Delegates, prioritizes, requests, and accepts guidance and assistance from others to assure delivery of care in a safe and timely manner.
  • Ensures own professional effectiveness through continued education and professional development.
  • Assists in transporting patients, specimens, supplies and equipment, and maintains and stocks supplies and equipment in patient homes.
  • Might triage patient phone calls, counsel patients, provide patient education, take patient histories, administer prescribed medications, change dressings, clean wounds, and monitor patient vital signs. Teaches patient and caregivers about disease management per plan of care.
  • Performs other duties as assigned.
Qualifications Education
  • Accredited State Board of Nursing LPN Program required.
Experience
  • Minimum of 1 year preferred: clinical experience as an LPN.
  • Preferred:
    Demonstrated knowledge of and ability to apply age‑specific principles of growth and development and life stages to meet each patient’s needs.
Credentials
  • Required upon hire:
    Licensed Practical Nurse – Oregon.
  • Required upon hire:
    Basic Life Support.
  • Required upon hire:
    Driver’s License and proof of insurance.
Skills
  • Basic computer skills required including familiarity with MS Office applications (Preferred).
  • Must have cooperative working and interpersonal skills and the ability to work effectively with a wide variety of patients, families, and staff (Preferred).
  • Able to communicate effectively with individuals diagnosed with mental and behavioral disorders, and operate efficiently in an independent home setting. Must be well organized, detail‑oriented, flexible, and able to function independently (Preferred).
  • Proficient with IVs (Preferred).
Working Conditions

Lifting:
Patient handling no greater than 35 lbs. without assistive equipment; fine motor skills required; push/pull up to 45 lbs; CPR 20 mins using 100‑125 lbs. of force; frequent sitting; bending, stooping, squatting, reaching, kneeling; lifting non‑patient up to 20 lbs occasionally.

Environmental conditions:

Exposure to biohazard, body fluids, and airborne particles; tasks must be completed in a noisy environment.

Mental/visual:
Vision and hearing within normal limits (glasses, contacts, hearing aids permitted); ability to communicate and exchange accurate information.
