Somatus is seeking an RN Care Manager, VBC, to support high-need CKD/ESKD populations. You will work with patients, nephrologists, PCPs, and care teams to coordinate in-home and clinic care, education, and resources.
The role involves travel to member homes and partner facilities, delivering care plans and coordinating with interdisciplinary teams to improve outcomes. A current RN license and BLS certification are required.
