Position:
Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN).

• Compensation:
  Starting at $34.00 per hour plus shift differentials

• Develop relationships with residents and families, supporting physical, spiritual, emotional, and clinical needs
• Assess, observe, evaluate resident clinical condition, document findings, and notify provider or RN when necessary
• Update or modify care plans to reflect changes in resident needs
• Supervise and lead care associates
• Administer medications timely and accurately according to orders
• Participate in meetings related to resident outcomes and care
• Respond to residents, families, or responsible parties promptly and professionally
• Participate in review of clinical indicator reviews—falls, wounds, nutrition, antipsychotic use, etc.
• Other duties as assigned by RN clinical leader

• Minimum of 1 year of experience in assisted living, home health, or long‑term care (preferred)
• Licensed as a Licensed Practical Nurse by state
• CPR certification required
• Passion to care for seniors and strong resident advocacy
• Experience with electronic medical records preferred
