This role focuses on working with clients across acute, chronic, and palliative needs, ensuring quality and realistic health goals. You'll be part of an interprofessional team, where your responsibility includes direct care, care plan development, and managing eligibility assessments—all crucial for client support within the community.
Key Responsibilities:
• Lead care initiatives for various health conditions
• Ensure development of effective care plans
• Assess client eligibility for services offered
• Facilitate communication among healthcare providers
• Deliver direct care and evaluate ongoing needs
Requirements:
• Graduate of a Licensed Practical Nurse program
• Active LPN registration with BC College
• Two years’ experience with geriatric and chronic patients
• At least one year in a supervisory role
• Valid BC Class "V" Driver’s License
