Integrated Care Clinician (Nurse) – the post holder will undertake telephone triage assessments of patients in an efficient way, advising the most appropriate intervention to meet the needs of the patient using best clinical practice and adhering to Trust Policy, protocols and guidelines.

The role is autonomous, assessing and advising callers within agreed guidelines.

Main duties of the job

As a Band 6 Triage and Assessment Nurse you will draw upon your clinical assessment skills to help and advise a broad spectrum of patients, from minor ailments and injuries to complex and urgent conditions, and hone your skills in a remote clinical decision-making environment.

You will have the autonomy to make decisions but be supported by our experienced team of senior clinicians, practice coaches and multi-disciplinary colleagues.

Person specification
  • NMC Registered Nurse (adult or child) with relevant clinical practice as a nurse in front line, acute or chronic care, or telephone based triage.
  • Evidence of undertaking regular update training and continuous personal/professional development as required by the regulating body.
