Job overviewRestormel Community Mental Health Team is looking for a motivated and experienced Mental Health Nurse to join our friendly and supportive team within Cornwall Partnership NHS Foundation Trust. This is an excellent opportunity to make a real difference to the lives of people experiencing mental health difficulties and to support their recovery through high-quality, person-centred care.
We are seeking a dynamic practitioner to lead our duty desk service. The role involves managing incoming telephone calls, triaging referrals, coordinating information and working closely with colleagues across the multidisciplinary team. You will need to be highly organised, able to prioritise effectively in a busy environment, and confident in making clinical decisions.
We welcome applications from experienced Band 6 staff and aspiring practitioners seeking career progression. A Band 5-6 developmental opportunity is available, with comprehensive support, supervision and training to enable successful progression to Band 6.

Main duties of the jobThis is a community-based role that may require frequent travel to various locations throughout the county including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements.
· Toberesponsiblefortheassessmentofcareneedsandclinicalmanagementofadefinedcaseload within the Cornwall Partnership NHS Foundation Trust (CFT) Mental Health Service.
· Tocarrycontinuingresponsibilityforthedevelopment,implementationandevaluationofprogramsofcare, utilising the care co-ordination system.
- To developtheinterfacebetweenprimaryandsecondarycareservicessothepatients/carersare offered focused and co-ordinated packages of care that are appropriate to their needs.

Person specificationEducation and Qualifications
- Current professional registration – NMC
- Extensive experience of working with mental health patients
- Knowledge around suicide awareness and prevention and the ability to support patients
You must have appropriate UK professional registration.
