We are seeking an experienced and dedicated Senior Social Worker to join our dynamic team

This is a community-based role that may require frequent travel to various locations throughout the county including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements.

• Todeveloptheinterfacebetweenprimaryandsecondarycareservicessothepatients/carersare offered focused and co-ordinated packages of care that are appropriate to their needs.

• Relevant professional registration – Social Worker
• Evidence of post basic training
• Current professional registration

• Extensive experience of working with mental health patients
• Knowledge around suicide awareness and prevention and the ability to support patients

You must have appropriate UK professional registration.
