Community Nurse - St Austell Community Nursing Team

We are looking for 2 caring and motivated Community Nurses for the St Austell Community Nursing Team. The role involves providing quality nursing care directly in patients’ homes, supported by a multidisciplinary team.

• Cornwall Foundation Trust
• Royal Cornwall Hospitals NHS Trust
• Cornwall and Isles of Scilly Integrated Care Board

• Assess, plan, implement and evaluate care without direct supervision.
• Provide evidence‑based care in line with competency and training records.
• Support patients, carers and relatives in their home environment to maximise independence and quality of life.
• Collaborate with the Community Nurse Team leader and other locality teams in a flexible manner.
• Travel frequently across the county, including areas not accessible by public transport.

• IT skills: use of Word, Excel, internet, recording systems.
• Flexibility and adaptability to meet job demands.

• Registered Nurse – Adult.
• Current Professional Registration with the NMC.
• Evidence of recent relevant professional development.
• Mentoring experience or willingness to pursue mentoring qualification and attend updates.
