The Role As a Community Nurse, you'll work independently with the support of an experienced multidisciplinary team, delivering high-quality nursing care to adults in their own homes. You'll be responsible for:
Completing holistic clinical assessments and developing personalised care plans Delivering safe, evidence-based nursing care and treatment Monitoring patients and responding to changing clinical needs Supporting and educating patients, families and carers Working closely with GPs and multidisciplinary teams Maintaining accurate clinical records and ensuring high standards of clinical governance About You To be considered, you'll need:
