Location: St. Austell

Referral Coordinator – Supported Housing

Location: St Austell, Cornwall

Salary: £15.34 per hour + Holiday pay = £17.19

Hours: 37 hours per week

Contract: Temporary – 2 months

Working hours: Monday–Friday, 9am–5pm

Start: ASAP

We are looking for an organised and experienced Referral Coordinator to join a supported housing service working with people fleeing domestic abuse who may have complex support needs.

Based in our St Austell office, you will be responsible for coordinating referrals into the service, working closely with professionals, partner agencies and the wider support team to ensure people are assessed and supported appropriately.

Key responsibilities
  • Coordinate and manage referrals into the service.
  • Liaise with referring agencies, professionals and partner organisations.
  • Gather and assess referral information to identify support needs and risks.
  • Complete initial risk assessments and ensure relevant information is shared appropriately.
  • Coordinate placements and liaise with the team regarding accommodation availability.
  • Maintain accurate referral records, case notes and databases.
  • Ensure referrals are progressed in a timely and organised manner.
  • Support the development and review of support plans where appropriate.
  • Identify safeguarding concerns and escalated risks appropriately.
  • Work closely with Support Workers to ensure a smooth transition into the service.
  • Support residents and professionals with move-on and tenancy-related referrals where required.
  • Manage competing priorities and maintain an organised workload.
About you

You will ideally have experience in referrals, supported housing, homelessness, domestic abuse, housing, social care, safeguarding or community services.

You must have:

  • Previous experience in a coordination, referral, housing or support-based role.
  • Excellent communication and organisational skills.
  • Experience working with confidential and sensitive information.
  • An understanding of risk assessment and safeguarding.
  • The ability to work effectively with multiple agencies and professionals.
  • Strong IT and administrative skills.
  • A Full Enhanced DBS covering Adults and Children or be willing to obtain one.
