Die Stelle umfasst das Erfassen der sozialen Situation beim Spitaleintritt, Beratung zu Wohn- und Lebenssituation nach dem Spitalaustritt sowie Organisation der Nachsorge. Zusätzlich erfolgen Finanzierungsberatungen und Informationen zum Sozialversicherungsrecht. Einleiten behördlicher Massnahmen gehört ebenso zum Aufgaben feld;
