EMS Mobile Integrated Health Program Coordinator Registered Nurse

Northeastern Vermont Regional Hospital St. Johnsbury, Vermont, United States

About this position

Help bring healthcare beyond the hospital walls and directly into the communities that need it most.

Northeastern Vermont Regional Hospital (NVRH) is seeking an EMS Mobile Integrated Health (MIH) RN Program Coordinator to support the Northeast Kingdom's innovative Mobile Integrated Health program. This unique role combines clinical expertise, care coordination, community partnership, and program leadership to help patients receive the right care, in the right place, at the right time. The coordinator serves as the central connection between patients, EMS agencies, healthcare providers, and community organizations throughout the Northeast Kingdom.

If you are passionate about population health, care coordination, community-based care, and improving healthcare access in rural communities, this is an opportunity to make a meaningful impact on the health and well-being of the region.

About This Role

As the Program Coordinator, you will oversee the day-to-day clinical and operational coordination of the Northeast Kingdom Mobile Integrated Health Program. Working closely with EMS field teams, patients, medical directors, healthcare organizations, and community partners, you will manage patients from referral through program completion while ensuring high-quality, coordinated care. Using your clinical expertise and nursing judgment, you will assess patient needs, coordinate services, support EMS field teams, facilitate community resource connections, and help drive quality improvement efforts that improve outcomes and reduce barriers to care.

Key Responsibilities

  • Coordinate patient enrollment, scheduling, care planning, follow-up activities, and ongoing program participation while maintaining the MIH patient census and monitoring progress toward care goals.
  • Serve as the primary liaison between patients, EMS agencies, healthcare providers, home health organizations, and community partners to ensure seamless communication, coordination, and continuity of care.
  • Support EMS field teams through clinical coordination, patient information review, identification of escalation needs, and coordination of appropriate follow-up for clinical and social determinants of health needs.
  • Co-lead quality improvement activities, monitor program outcomes and documentation quality, support grant reporting requirements, and participate in ongoing program development and evaluation efforts.

Required Qualifications

  • Current, unrestricted Vermont or a Multistate Compact Registered Nurse (RN) license required.
  • Minimum three years of clinical experience in emergency nursing, EMS, critical care, community-based care, or a comparable clinical setting requiring independent clinical judgment.
  • Experience with patient care coordination and multidisciplinary collaboration.
  • Strong clinical assessment, communication, organizational, and problem-solving skills.
  • Ability to manage multiple priorities independently and maintain accurate documentation.
  • Proficiency with electronic health records and documentation systems.
  • Valid driver's license required.

Preferred Qualifications

  • BSN preferred; equivalent education and relevant clinical experience may be considered.
  • Experience in Mobile Integrated Healthcare (MIH), community paramedicine, home health, care management, or population health.
  • EMS, Emergency Department, or Critical Care experience.
  • Program coordination or leadership experience.
  • Rural healthcare or Critical Access Hospital experience.
  • Familiarity with Vermont EMS systems and Northeast Kingdom healthcare resources.
