Are you a highly skilled and effective advanced clinical practitioner? Dorset Integrated Urgent Care Service (DIUCS) is seeking an experienced advanced clinical practitioner to provide clinical leadership and expertise in our multi‑professional team.
As an established autonomous practitioner with highly developed communication skills, you will make complex clinical decisions, plan and manage comprehensive assessments of patients across Dorset's diverse population.
Using your advanced clinical skills in developing and implementing the highest quality clinical assessment and treatment, you will work across IUCS teams, including assessing patients over the telephone or video consultation, managing patient referrals through the Single Point of Access Service (SPoA), visiting patients face‑to‑face with the out‑of‑hours service and urgent community response service (UCR), with a potential to work in the IUCS Treatment Centres.
This is a part‑time position. We welcome applications with a minimum of 23 hours per week based at St Leonards HUB. The role will cover evenings, weekends and bank holidays as part of a 24/7 service. Twelve‑hour shifts are included within the shift pattern. Independent non‑medical/supplementary prescriber.

Main duties of the job- Independent non‑medical/supplementary prescriber, able to take a history, assess, examine, diagnose and prescribe a management plan including medication and monitor response to medication in compliance with national prescribing standards.
- Proactive in clinical decision‑making, demonstrating a high level of knowledge in relation to patterns of disease/disorder. Able to assess critically and re‑evaluate the clinical situation as the patient’s condition or needs change.
- Establish, maintain and effectively manage barriers to advanced, highly skilled and effective communication. Collect, collate, evaluate and report information, maintaining accurate confidential patient records.
- Develop, implement and audit protocols, guidelines and policies and identify own personal developmental and educational needs. Also undertake clinical supervision and systematic peer caseload review.
- Assess patients using telephone consultation and video consultation.
- Assess and treat patients in the community as part of the out‑of‑hours and Urgent Community Response teams. Work alongside the SPoA service, supporting the wider system with alternative pathways for patient care needs.
Employment in this post requires an Enhanced/Standard Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of.
Person Specification Knowledge, Skills and Training- Registered health practitioner with membership of relevant professional body.
- Demonstrable knowledge of legislation, guidelines and best practice relevant to the area of clinical practice and application of these to practice.
- Evidence of knowledge and application of national work and standards in relation to advanced clinical practice and the relevant specialty.
- Clear and demonstrable evidence of advanced, autonomous practice in the management of patients/service users.
- Knowledge and understanding of the Trust’s strategies relevant to the role.
- Demonstrable experience of working in a provider organisation and within a relevant specialty as a registered practitioner Band 7 level.
- Demonstrates highly developed expertise and knowledge underpinned by theory acquired through CPD in relation to clinical assessment, treatment and evaluation.
- Experience of coordinating complex activities/programmes of care with other professionals and agencies.
- Evidence of involvement in policy and leading practice change.
- Experience of interdisciplinary working.
- Able to demonstrate advanced skills for assessing and interpreting complex client conditions, comparing a range of options and taking appropriate action.
- Trained in PMVA techniques or willing to be trained (as per role requirements).
