Responsibilities
  • Provide professional nursing care to patients in their homes after normal business hours or weekends, through assessment, development, implementation, and evaluation of home nursing care needs.
  • Assure compliance with physician orders in a timely manner and ensure documentation reflects compliance and progression toward goals.
  • Ensure assessment, planning, intervention, and evaluation of patient care follow policy, procedure, and standard of care.
  • Demonstrate ability to care for a patient and significant other in a holistic manner and provide safe patient care.
  • Utilize an interdisciplinary team and accurately document all activities.
  • Make clinical judgments and apply critical thinking reflecting professional standards of care and codes of ethics.
  • Document clinical decisions appropriately.
  • Identify appropriate payors, comply with authorizations, maintain visit frequency, and use secure messaging to necessary contacts.
  • Complete nursing assessment, medication reconciliation, and collaborate with physician on Plan of Care.
  • Update care plans within two weeks of changes in patient status.
  • Implement teaching strategies to meet established goals on the plan of care and document them.
  • Report patient care problems to manager or appropriate personnel in a timely manner with solutions.
  • Act as liaison among the physician, Manager of Clinical Practice, and other health care team members to ensure effective coordination.
  • Communicate pertinent patient information and Plan of Care during transfers of care loads or schedule changes.
  • Complete routine assignments accurately and consistently without the need for supervision, meeting productivity requirements.
  • Serve as clinical supervisor of para‑professional home health aide staff, completing supervisory visits within required time frames and contributing to staff evaluation.
  • Maintain awareness of updated standards and incorporate them into nursing practice.
  • Adhere to HMH's organizational competencies and standards of behavior.
  • Lift a minimum of 50 lbs., push and pull a minimum of 50 lbs., and stand a minimum of 8 hours a day.
Qualifications
  • Associate's Degree in Nursing or Diploma Certificate in Nursing.
  • Minimum of six months RN experience or one year LPN experience, preferably in home care or hospice.
  • Excellent written and verbal communication skills.
  • Proficient computer skills (Microsoft Office and/or Google Suite).
Education &

Preferred Qualifications
  • Bachelor's of Science in Nursing.
Licenses & Certifications
  • NJ State Professional Registered Nurse License.
  • AHA Basic Health Care Life Support (HCP) Certification.
