• Assist patients in their home after normal business hours or weekends by assessing, planning, implementing, and evaluating individualized nursing care plans.
• Ensure compliance with physician orders, timely documentation, and progression to discharge goals.
• Follow policy, procedure, and standard of care during assessments, interventions, and evaluations; document all activities accurately.
• Apply clinical judgment consistent with professional standards, codes of ethics, and safe patient outcomes.
• Identify payors, secure authorizations, maintain visit frequency within authorization ranges, and use secure messaging for insurance communications.
• Complete assessments, medication reconciliation, and collaborate with physicians on plan of care; update care plan within two weeks of status changes.
• Implement teaching strategies aligned with care plans; document teaching activities in the chart.
• Report patient care issues to manager or appropriate personnel with potential solutions; ensure documentation reflects case communication.
• Serve as liaison among physicians, clinical managers, and the health care team to coordinate services and meet client needs.
• Communicate relevant patient information and plan of care during transfers, whether daily or as needed for vacation days.
• Complete routine assignments accurately, consistently, and independently, meeting productivity requirements.
• Supervise para‑professional home health aide staff; conduct supervisory visits within regulatory time frames and document visits.
• Stay current with updated standards, incorporating them into practice; demonstrate knowledge of best practices in the medical chart.
• Complete other duties/projects as assigned and demonstrate flexibility across all job areas.
• Adhere to HMH's organizational competencies and standards of behavior.
• Lift a minimum of 50 lbs., push and pull a minimum of 50 lbs., and stand for a minimum of 8 hours per day.

• Associate's Degree in Nursing or Diploma/Certificate in Nursing.
• Minimum 6 months RN experience or 1 year LPN experience, preferably in home care or hospice.
• Excellent written and verbal communication skills.
• Proficient computer skills (Microsoft Office and/or Google Suite).

• Bachelor's of Science in Nursing.

• NJ State Professional Registered Nurse License.
• AHA Basic Health Care Life Support (HCP) Certification.

Minimum annual rate: $88,441.60. The posted rate is a reasonable good‑faith estimate of the minimum base pay and is subject to change based on experience, education, certifications, and other market factors.
