A summary is as follows:
Take a Recovery focused and holistic approach in working with the people who access our services.

Deliver care and interventions utilising the stepped care model, including provision of the CPA Framework where appropriate.

Assess people's needs to develop, implement and evaluate personalised care plans.

Facilitate therapeutic group work taking the lead where appropriate.

Deliver evidenced based interventions.

Identify people requiring hospital treatment and facilitate the timely transfer under The Mental Health Act.

Line management of Band 5 nurse where appropriate.

Some weekend working is required as part of this role.
