An opportunity has become available in the Integrated Discharge Hub (IDH) for a Band 4 Integrated Health & Social Care Assessor D2A. We need a resilient and dynamic individual to join the experienced and enthusiastic team.

The work is challenging and involves liaising with patients, their family's and ward professionals via telephone or face to face as appropriate to ensure gathering of accurate information in order to facilitate effective, safe and timely discharge from the acute hospital.

Main duties of the job

The successful applicant will be part of a supportive, multi-disciplinary team within a high pressured environment so will need to be able to work at pace and be able to move between competing priorities.

You need to be an excellent communicator, have strong written and verbal skills and be able to transfer information accurately onto a Transfer of Care form to identify patient needs and formulate effective person centred outcomes to support effective patient transfer.

This is an ideal opportunity for any individual who has a knowledge of the Care Act 2014 and who wants to work within an unplanned care setting and access a variety of training via the social work learning academy (SWLA)

On appointment, the post holder will be auto-enrolled in the SWLA and aligned to the appropriate British Association of Social Work Professional Capabilities Framework. The relevant SWLA stage (i.e. stage 1 (ASYE), 2 (Social Worker) or 3 (Advanced Social Worker) will also be agreed taking all prior Continuous Professional Development (CPD) and relevant experience into consideration. The post holder will be required to take responsibility for their own Continuous Professional Development.

Person Specification QUALIFICATIONS & TRAINING
