Stafford Health & Wellbeing Centre is seeking an enthusiastic and motivated Practice Nurse to join our friendly, patient-focused primary care team for 25 hours per week.

As a valued member of our multidisciplinary team, you will play a key role in delivering high-quality nursing care to our diverse patient population. The successful candidate will undertake a range of responsibilities including chronic disease management, cervical screening, childhood and adult immunisations, health promotion, wound care, and supporting patients to achieve positive health outcomes.

Main duties of the job

Key Duties and Responsibilities

As a Practice Nurse, you will provide high-quality, evidence-based care within a busy NHS primary care setting, working closely with GPs, ANPs, PAs, an HCA and the wider multidisciplinary team to deliver safe, effective and patient-centred services.

Key responsibilities include:

  • Managing long-term conditions such as diabetes, asthma, COPD, hypertension and cardiovascular disease.
  • Undertaking Infection Prevention and Control (IPC) activities in line with national and local standards.
  • Delivering routine, travel and seasonal vaccinations, including flu and COVID-19 programmes where required.
  • Performing cervical screening in accordance with NHS guidelines.
  • Providing health promotion, lifestyle advice and preventative care to improve patient wellbeing.
  • Managing wound care, dressings, leg ulcers and suture removal.
  • Supporting anticoagulation monitoring, medicines reviews and clinical observations.
  • Carrying out NHS Health Checks, screening and other preventative health initiatives.
  • Maintaining accurate clinical records and contributing to audits and quality improvement activities.
  • Supporting safeguarding, infection control, patient safety and clinical governance requirements.
  • Assisting with care planning, patient education and self-management for those with complex or long-term conditions.
  • Working collaboratively with colleagues to ensure coordinated, high-quality patient care.
