Senior Staff Nurse - Community Intervention Service

You will:

• Be a Registered Nurse with current NMC registration.
• Hold a Degree in Nursing or equivalent qualification.
• Hold postgraduate study in a relevant area such as Long-Term Conditions Management, or Advanced Clinical Practice at level 7 study.
• Have experience working within community nursing.
• Demonstrate excellent communication and interpersonal skills, with the ability to build effective relationships with patients, carers, and multidisciplinary teams.
• Be confident in working independently and managing a diverse caseload while prioritising workload effectively.
• Possess strong clinical assessment, problem-solving, and decision-making skills.
• Have experience supporting hospital discharge processes and admission avoidance pathways.
• Be committed to delivering safe, effective, and compassionate patient care.
• Demonstrate leadership qualities and a willingness to support, mentor, and develop colleagues.
• Show a commitment to continuous professional development and lifelong learning.
