We are looking for an experienced and motivated Specialist Neighbourhood Nurse to join our dynamic and working closely with the Neighbourhood team and practice teams.
Youll be involved in the smooth day to day running of housebound and care home patients engaging in assisting both practices to achieve its vision and objectives.
Neighbourhood Nurses working in primary and community care bring a wide range of clinical skills, a strong generalist knowledge base, and the ability to deliver high-quality, person-centred care in patients homes and community settings. Attimes, this care will be supported by specialist professionals in both primary and secondary care, with effective use of diagnostic, screening and referral pathways. The post holder will work collaboratively with colleagues within the Neighbourhood team and across wider services to ensure coordinated and seamless care.
The SSN Specialist Neighbourhood Nurse will be expected to demonstrate:
- The ability to make professional autonomous decisions within their level of competence and accountability.
- The ability to assess,plan, deliver and evaluate nursing care for patients with a range of undifferentiated, complex and long-term conditions in the community.
- Competence in screening for risk factors and identifying early signs of deterioration orillness.
- Strong clinical reasoning and problem-solving skills to support safe decision-making and escalation where required.
- The ability to develop, in partnership with patients and carers, individualized care plans with an emphasis onhealth promotion, self-management and prevention

About the Practice/Department
As a SpecialistNeighbourhood Nurse, you will play a key role in delivering high-quality, patient-centredcare to individuals in their own homes and community settings. You will support patients with long-term conditions, frailty, wound care, end-of-life care and complex health needs, helping to prevent unnecessary hospital admissions and promote independence.
Job responsibilitiesThe role of the Specialist Neighbourhood Nurse is to support both the practice staff and members of the Neighbourhood Team to identify and support people to reduce therisk of unplanned hospital admissions and to effectively support those individuals in the community.
Person Specification Qualifications- Registered Health or Care Professional
- Post graduate study in health-related studies relevant to long term conditions or equivalent experience.
- Evidence of continuing professional development
- Post registration teaching qualification or willingness to undertake
- Experience of dealing with people with long term conditions.
- Experience of working within a multidisciplinary team
- Experience of teaching or mentorship
- Experience of complex case management and multiagency working
