As a Long-term conditions nurse, you will provide and maintain a high standard of long-term disease management, which includes diagnosis, monitoring & development of individual management plans,agreeing these as appropriate with the patient & other health professionals.
Main duties of the jobYoull be involved in the smooth day to day running of the Long-Term Condition Clinics.
Duties may include but are not limited to:
- Clinical Assessment & Monitoring

- Patient Education & Self-Management Support
- Prevention & Health Promotion

About the Practice/Department/Team
Lakeside Healthcare Group, Stamford surgery is aforward thinking, progressive organisation offering primary care services tothe people of Stamford and surrounding villages. We are a large practice with apopulation of 31,000 patients.
Job responsibilitiesSpecifically in Diabetes, CHD and HF (asthma/COPD mayalso be part of the role); you will work strictly in accordance with specific practice guidelines & protocols. Working within the NMC Code of Conduct and professional boundaries, the post holder will deliver primary health care interventions and care. The post holder will support service development, the safe delivery of policy and procedures, and demonstrate effective professional nurse standards, advocacy, and performance.
PersonSpecification Qualifications
- Registered Nurse
- Evidence of continued professional development
- Accredited Diploma Level Award in chronic disease management or equivalent
- Primary Care Nursing qualification or equivalent
- Mentorship course or equivalent
- Experience of managing/dealing with the public/patients
- Experience of working within healthcare governance systems and understanding role and responsibilities for CQC and within risk management
- Experience of using clinical systems & Microsoft Office software
- Experience in Primary or Community Care Nursing or equivalent relevant experience
- Experience of audit and/or research
- Experience of Systm One
- Experience of group teaching and 1:1 teaching to support patients with
