We are looking for an experienced and motivated Advanced Nurse Practitioner to joinour dynamic and working closely with the leadership and practice teams. Youllbe involved in the smooth day to day running of the practice and pro-actively engaged in assisting both practices to achieve its vision and objectives.

ANPs operating inprimary care have a wide range of skills, a broad knowledge base, and the ability to deliver specific aspects of care. At times these will need to be supplemented by the skills of specialist health care professionals in both primary and secondary care and include use of a range of diagnostic and screening services. Although independent practitioners, the ANP act collaboratively with colleagues working in the same area of practice or referto and share care with colleagues in more specialist areas.

In summary, thepost holder is an experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. Theywill demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice.

The post holder will demonstrate critical thinking in the clinical decision-making process.

They will work collaboratively with a range of multi-professional teams to meetthe needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. To work at this level, NMC requirements for advanced practice must be met.

Supporting the overarching Lakeside Healthcare Group strategy and service delivery, the Nurse Practitioner (NP),Emergency Care Practitioner (ECP), Advanced Nurse Practitioner (ANP) provides complete episodes of care for patients with a wide variety and range of presenting problems andhealth care needs. This encompasses the provision of evidence based,high-quality care for patients whose issues fall within urgent/acute episodes, long-term/chronic conditions and including health promotion and public health elements.

• Educated to Masters Level Nurse Practitioner Pathway
