Stamford Health has an opportunity for a full-time Clinical Oncology Pharmacist in Stamford, CT! The Clinical Oncology Pharmacist will work Monday – Friday 8-4 / 9-5.

- Improves patient‑centered care and medication outcomes by supporting the oncology team, providers, and patients, engaging in quality initiatives.
- Ensures safe medication practices by focusing on treatment appropriateness, effectiveness, safety, cost awareness, and adherence.
- Serves as the oncology subject matter expert (SME) to provide insights for formulary selection, clinical pathways, medication policies, and guidelines.
- Collaborates with the Pharmacy Informatics Specialist and IT pharmacy analysts to address EHR upgrades, testing, and modifications.
- Participates in pharmacy and Bennett Cancer Center EHR work groups to develop, review, and implement new medications and treatment plans.
- Performs final EHR validation as the SME of new medication treatment plans in collaboration with clinicians.
- Reviews new medication and participates in treatment plan validations.
- Brings medication requests that do not meet criteria to the Pharmacy & Therapeutic Committee for approval.
- Promotes safe practice by minimizing medication errors in ordering, preparation, and administration for outpatient and inpatient settings.
- Writes, reviews, and manages all new or modified chemotherapy and biotherapy clinical policies.
- Ensures treatment regimens align with clinical pathways and NCCN approvals.
- Acts as pharmacy liaison to align chemotherapy policies between Bennett Cancer Center and inpatient pharmacy and nursing services.
- Performs other related duties as assigned.
- PGY2 residency in oncology or PGY1 residency with 3‑5 years of clinical experience in an oncology practice setting.
- Board Certification in Oncology (BCOP), preferred on hire or within 1 year.
- Graduate from an accredited College of Pharmacy.
- Doctor of Pharmacy degree.
- Licensed Pharmacist (or eligible for licensure) in the State of Connecticut.
- Maintains all licensure/registration requirements applicable to the role.
- Possesses knowledge of complex anticancer therapy regimens and safe prescribing.
- Excellent verbal and written communication, time‑management, and collaboration skills.
