What Makes This Opportunity Unique?Greenwich Hospital's Department of Psychiatry is seeking an experienced Advanced Practice Registered Nurse (APRN) to support the Adult Intensive Outpatient Program (IOP) located at 2015 West Main Street. The Adult IOP serves patients with high-acuity behavioral health needs who are transitioning from inpatient psychiatric hospitalization to a less restrictive level of care.
This role is integral to the interdisciplinary treatment team and is responsible for providing comprehensive psychiatric evaluation, medication management, and ongoing clinical support to patients participating in the program. The successful candidate will work collaboratively with psychiatrists, therapists, nursing staff, and other behavioral health professionals to promote stabilization, recovery, and successful community reintegration.
Essential Responsibilities- Provide psychiatric assessments and ongoing evaluation of adult behavioral health patients enrolled in the IOP.
- Manage psychotropic medications, including prescribing, monitoring effectiveness, and adjusting treatment plans as clinically appropriate.
- Develop and implement individualized treatment plans in collaboration with the multidisciplinary care team.
- Monitor patient progress, symptom management, and treatment outcomes.
- Provide patient and family education regarding diagnoses, medications, and treatment recommendations.
- Coordinate care with inpatient psychiatric units, outpatient providers, primary care clinicians, and community resources.
- Ensure timely and accurate documentation in accordance with regulatory, organizational, and departmental standards.
- Participate in program development, quality initiatives, and interdisciplinary case reviews.
Required
- Active Connecticut APRN license.
- Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP) or equivalent psychiatric APRN certification.
- Minimum of 1-2 years of independent clinical practice experience in behavioral health or psychiatry.
- Demonstrated experience with psychiatric assessment, diagnosis, and medication management.
- Strong communication, collaboration, and clinical decision-making skills.
Preferred
- Experience working in intensive outpatient, partial hospitalization, inpatient psychiatry, or other acute behavioral health settings.
- Experience caring for patients transitioning from inpatient psychiatric hospitalization to outpatient treatment.
- Monday-Friday
- Day and evening hours
- 37.5 hours per week
