Integrated Clinical Team Manager - South Uttlesford

The post holder will lead the Uttlesford South Integrated Team, working closely with Community Nursing and local partners to deliver high-quality, evidence-based care in the community. They will provide day-to-day operational leadership, line management and clinical supervision, ensuring safe, effective and integrated service delivery.

Working collaboratively with the North Uttlesford Team Lead, the South Uttlesford PCN, Social Care, the Virtual Hospital and the Care Coordination Centre, they will support admission avoidance, timely discharge and care delivered in the most appropriate setting. The role focuses on driving integration, service improvement and the implementation of clinical and managerial strategies across the locality.

Main duties of the job
  • Lead and manage the Uttlesford South Integrated Team to deliver high-quality, evidence-based community care.
  • Provide day-to-day operational leadership, including line management and clinical supervision.
  • Ensure safe, effective and integrated service delivery across the locality.
  • Work collaboratively with the North Uttlesford Team Lead to maintain cohesive leadership across North and South teams.
  • Build strong partnerships with the South Uttlesford PCN, Community Nursing, Social Care, the Virtual Hospital and the Care Coordination Centre.
  • Support admission avoidance, timely discharge and care delivered in the most appropriate setting.
  • Contribute to service integration and continuous improvement initiatives.
  • Support Operational Managers in implementing and monitoring clinical and managerial strategies.
  • Use population health information to inform service development and meet local needs.

Job responsibilities Clinical Responsibilities
  • Work within a Caseload Management Framework to coordinate care using clinical judgement and partnership with the wider Primary Health Care Team.
  • Identify and plan preventative and anticipatory care needs.
  • Maintain accurate, contemporaneous patient records and update relevant professionals on changes in condition.
  • Proactively monitor patients and review care plans regularly.
  • Make referrals, request diagnostic tests and initiate treatments or interventions as required.
  • Assess patients with complex needs using advanced clinical examination, history taking and evaluation skills.
  • Manage End of Life Care in partnership with health, social care and voluntary agencies.
  • Prepare and support patients and families for changes in condition and end-of-life choices.
Organisational Responsibilities
  • Work in accordance with EPUT policies, procedures and guidelines.
  • Ensure timely reporting of accidents, incidents and near misses.
  • Sign off incidents on DATIX within 10 working days, following Trust policy.
  • Submit statistical and contractual monitoring returns within agreed timescales.li>
  • Ensure Mandatory and Statutory Training is completed within required time frames.
  • Manage annual leave in line with Trust policy and SOPs.
  • Participate in clinical supervision.
