Als casemanager dementie bij RST Zorgverleners in Staphorst ben je de steun en gids voor cliënten en hun naasten in complexe zorgsituaties. Je organiseert passende zorg, coördineert zorgverleners en biedt ondersteuning om zelfstandig wonen te faciliteren.
Wij vragen een HBO-verpleegkundige of iemand gemotiveerd voor opleiding in casemanagement dementie. Wij bieden een aantrekkelijk salaris volgens CAO VVT, inclusief vakantietoeslag, eindejaarsuitkering en 33 vakantiedagen.
