Speech Language Pathologist (Home Health) (Part time/PRN)
Join Medfamily Wellcare Center as a licensed Speech Language Pathologist to assess, diagnose, and treat communication and swallowing disorders in patients' homes.
Responsibilities- Conduct initial assessments and develop individualized treatment plans.
- Collaborate with the care team to ensure continuity of care.
- Document patient progress in health records.
- Focus on cognitive-communication and swallowing issues resulting from stroke, brain injury, illness, surgery, injury, or dementia.
- Current SLP license.
- Valid driver’s license.
- Ability to relocate to State College, PA before starting.
