Job Description

The Clinical Performance Nurse (CPN) is a member of the hospitalist team who is responsible for delivering processes designed to improve quality and efficiency in the care of hospitalized patients. The CPN works closely with the Medical Director and Practice Manager to engage and educate the physician team, optimize performance on defined key metrics, and ensure collaboration with the care team.

This position does not involve providing direct patient care.

Position Summary

The CPN oversees and facilitates clinical performance activities across the full acute episode of care, including identifying patient cohorts for initiatives, participating in daily workflows, advocating for patient goals in care plans, and ensuring communication throughout patient care. Success will be measured by improved patient outcomes, experience, and resource utilization.

Minimum Requirements
  • Completion of a registered nurse degree program required.
Experience
  • 3‑5 years acute care nursing experience.
  • Experience in one or more of the following is preferred:
    Case Management;
    Utilization Management;
    Quality Improvement;
    Performance Improvement; regulatory and compliance processes.
  • 1‑2 years experience in a nursing leadership role would be a plus.
Knowledge, Skills, Abilities
  • Effectively functions as a team member and as a liaison between patient and staff.
  • Strong clinical knowledge and experience in the Acute Care Hospital setting (Critical Care, Emergency Department, and/or Case Management experience is helpful).
  • Knowledge of process improvement methodology.
  • Exceptional customer service and demonstrated effective interaction with physicians, patients, staff, and outside agencies.
  • Strong verbal, non‑verbal, and written communication skills.
  • Professional communication at all times.
  • Ability to assess patient’s clinical status and readiness for discharge.
  • Ability to work independently and problem‑solve based on patient needs.
  • Excellent organizational skills with the ability to multi‑task and prioritize workload in a fast‑paced environment while maintaining attention to detail.
  • Demonstrates a high level of mental and emotional tolerance and even temperament when dealing with ill people; uses tact, sensitivity, sound judgment and a professional attitude when relating with patients, families, and co‑workers at all times.
  • Continually demonstrates poise, tact, diplomacy, and good judgment.
License / Certification / Registration
  • Licensed as a Registered Nurse in the State of Pennsylvania.
  • Meets the American Heart Association’s standards for Healthcare Provider Basic Life Support (BLS) and renews BLS every two years.
Supervision
  • Receives general supervision from the Manager and Medical Director of Hospitalist Medicine.

Responsibilities ESSENTIAL FUNCTIONS – Clinical Performance
  • Participate in the development of site‑specific workflows to achieve program goals.
  • Partner with the Medical Director and Practice Manager to implement processes and lead process optimization.
  • Provide timely status of implementation, barrier resolution, and action plans.
  • Understand, clearly articulate, and promote the adoption of hospital clinical performance objectives with the physician team.
  • Engage and provide education to the provider team(s) related to clinical performance initiatives, goals, and measurement.
  • Facilitate coordination between the hospitalist team and hospital departments (nursing, quality, case management, clinical documentation) to ensure the delivery of workflows related to quality measure performance, throughput, discharge facilitation, transitions of care, readmission prevention, patient experience, timely physician response to documentation queries, and custom initiatives.
  • Collaborate with the team to establish and communicate patients’ care goals.
  • Provide patient and family education to ensure understanding of the disease process, next site of care options, and community resources.
  • Establish a process for the completion of Home Health Care assessments to determine eligibility when applicable.
