Occupational Therapist (OT)

- Evaluate referred residents to determine their functional level and potential for rehabilitation.
- Provide direct care and develop individualized treatment plans to help residents regain independence in activities of daily living (ADLs).
- Establish treatment goals based on each resident’s abilities, limitations, and physician recommendations.
- Implement therapeutic interventions to improve strength, coordination, mobility, cognition, and self-care skills.
- Educate residents and their families on home programs and adaptive techniques.
- Complete documentation including evaluations, weekly progress notes, recertifications, and discharge summaries.
- Collaborate with the interdisciplinary team to ensure a resident-centered approach to care.
- Adhere to all applicable federal and state regulations, including HIPAA and Residents' Rights.
- Perform additional duties as assigned by the Director of Rehabilitation.
Qualifications:
- Graduate of an accredited Occupational Therapy program.
- Valid Occupational Therapy license and registration for the state of practice.
- Experience in long-term care or geriatrics is preferred but not required.
- Strong communication and organizational skills.
- Compassionate, team-oriented, and committed to resident-centered care.
