We are recruiting Social Care Practitioners to work in our established Community Hospital and Community Support Bed Assessment and Discharge Team.
Our 6 Community Hospitals and 3 Community Support Beds offer short‑term assessment and reablement / rehabilitation to people across Derbyshire, helping them to regain confidence and maximise independence. They support people leaving hospital, requiring urgent community response or benefiting from reablement in a bedded care setting (Derbyshire’s ‘Pathway 2’ / intermediate care service).
These sites are supported by skilled multidisciplinary teams from health and social care. You will work with a team of professionals to contribute to goal setting and discharge planning, complete Care Act assessments and support plans, and support people within the Mental Capacity Act and safeguard personal data.
The services are undergoing an exciting period of change over as we transition to a new partnership between Derbyshire County Council and Derbyshire Community Health Services NHS. The roles will transition into this partnership around 2026.
The majority of your time will be spent at one of our Community Support Bed or Community Hospital sites, with some travel to other sites and hybrid working to meet service needs. Two positions focus on the Chesterfield area (with flexibility to cover other areas), the third position is adaptable across the wider Derbyshire region.
Key responsibilities- Provide person‑centred and strength‑based assessments
- Deliver timely discharge planning support
- Apply the Care Act 2014, including a good understanding of Making Safeguarding Personal, and the Mental Capacity Act
- Work as a team player alongside other disciplines and teams
- Work autonomously with a positive, can‑do attitude
