Primary Care Advanced Practice Provider — Stayton, Oregon We are seeking an experienced Nurse Practitioner or Physician Assistant to join a well-supported outpatient clinic in Stayton, Oregon. • Schedule:
  
  40 hours/week, salaried.
• Team-Based Care:
  Work with medical assistants, behavioral health clinicians, care coordinators, and others.
• Focus:
  Provide primary care tailored to the clinic’s patient population.
• Initiatives:
  Engage in quality improvement, virtual care integration, and patient education.
• Technology: EPIC EMR system.

• Active NP or PA license in Oregon.
• Minimum 2 years’ experience in Family or Internal Medicine.
• Current DEA and BLS or ACLS certification.

• Competitive base salary plus wRVU bonus potential.
• Sign-on bonus and relocation assistance.
• 34 days PTO annually (includes vacation, CME, holidays).
• $3,000 CME allowance.
