Key Responsibilities:

• Assess individualized needs of each patient effectively

• Execute and adjust personalized care plans responsibly

• Build rapport with patients through respectful engagement

• Be available for on-call responsibilities as required
Requirements:

• Active registration with the CNO as a Registered Nurse

• Valid driver’s license and vehicle access necessary

• Preferred experience in community nursing

• Willingness to travel within assigned areas

• Reliable with flexible scheduling
