Position Summary
The Nurse Practitioner (NP) plays a critical role in providing comprehensive primary healthcare services, with a specialized focus on refugee health and pediatric care. The Nurse Practitioner will maintain and manage their own roster of patients, working within a multidisciplinary team to deliver high-quality, client-centered care. This role requires a commitment to providing accessible healthcare services and working collaboratively to improve health outcomes for vulnerable populations.

Main Responsibilities/Duties

Provide comprehensive primary healthcare services to individuals and families, with a focus on refugee and pediatric populations.

Conduct health assessments, diagnose conditions, and develop treatment plans in accordance with best practices and clinical guidelines.

Prescribe medications and therapeutic interventions within the NP scope of practice.

Manage and coordinate care for patients with chronic and complex health conditions.

Collaborate with an inter-professional healthcare team, including physicians, nurses, social workers, and community health professionals.

Provide patient education, counseling, and advocacy to promote health and well-being.

Support the development and implementation of health programs tailored to the needs of refugee and pediatric patients.

Maintain accurate and timely electronic medical records (EMR) and documentation.

Participate in quality improvement initiatives and professional development activities.

Support and contribute to our company’s commitment to workplace equity, diversity, and inclusion.

Education and Employment Requirements

Successful completion of a post-graduate Primary Care Nurse Practitioner program.

Current Extended Class Registration with the College of Nurses of Ontario.

Proof of current membership or alternative professional liability coverage.

Current CPR certification (Level C or HCP).

Demonstrated experience in community and/or primary care.

Experience in chronic disease management and prevention.

Strong ability to work both independently and collaboratively within a healthcare team.

Excellent oral and written communication skills in a clinical setting.

Proficiency in the use of electronic medical records (EMR);
Accuro experience is an asset.

Passion and experience in Refugee Health and Pediatric Care are considered strong assets.

Compensation & Benefits

$52.10 - $68.41 per hour + on-call stipend

Full-Time (37.5 hours per week) Part-time arrangements will be considered.
