As a Nurse Practitioner, you will manage comprehensive care plans and provide leadership to clinical staff.

Your role will involve critical thinking for assessing and treating residents, while also engaging in continuous quality improvement initiatives. We're looking for a compassionate individual committed to optimizing resident health.

Key Responsibilities:

• Deliver primary care and monitor patient wellbeing

• Participate in care planning and assessments

• Coordinate with healthcare teams for resident support

• Educate teams on clinical best practices

• Lead initiatives to enhance palliative care services

Requirements:

• Valid Nurse Practitioner registration with CNO

• Preferred experience in long-term care

• Excellent interpersonal skills essential

• Completion of Vulnerable Sector Check

• Required vaccination documentation mandatory
