Elevate community health services with Care Partners as a Personal Support Manager. This temporary full-time role brings team coordination and community health management into focus.

As part of Care Partners' dynamic Leadership Team, the Personal Support Manager will coordinate and supervise Community Support Workers while ensuring high-quality patient care. This role requires creating effective care plans and staying updated on regulations and best practices in community health services.

Key Responsibilities:

• Supervise and coach Community Support Workers in the field

• Develop and maintain effective Care Plans for patients

• Monitor employee performance and adherence to policies

• Market Community Health Services to enhance public relations

• Guide workplace Health and Safety initiatives

Requirements:

• Registered Nurse (RN) or Registered Practical Nurse (RPN) certification

• 3-5 years of relevant job experience

• Registration with the College of Nurses of Ontario

• Proven leadership experience preferred

• Current CPR Certification and clear background check
