Position: Licensed Practical Nurse (LPN) for Patient Care

Key Responsibilities:

• Deliver direct nursing care according to facility policies

• Administer prescribed medications and treatments

• Monitor patient conditions and document vital signs

• Assist in patient admissions and discharges

• Collaborate with various healthcare professionals on care plans

Requirements:

• Active Licensed Practical Nurse (LPN) license needed

• BLS certification is required; ACLS preferred

• At least 1 year experience in Long Term Care

• Experience in healthcare settings like skilled nursing

• Familiarity with EPIC systems
