About the Role

Our client is seeking a dedicated Community Care Facilitator to support residents in Paarl, Western Cape . This role is pivotal in ensuring individuals and families have access to the social services and support networks they need to thrive. You will act as a key link, connecting community members with appropriate resources and facilitating their engagement with local support systems. This hybrid position balances essential on-the-ground community interaction in Paarl with remote administrative duties, offering a dynamic and fulfilling career path focused on social impact.

• Assess the social and emotional needs of community members and provide appropriate support.
• Facilitate access to social welfare programs, healthcare services, and community resources.
• Develop and maintain positive relationships with community members, local organizations, and service providers.
• Organize and coordinate community-based support groups and activities.
• Maintain accurate client records and prepare regular progress reports.
• Advocate for the needs of clients within various service delivery systems.

• A relevant qualification in Social Work, Community Development, or a related discipline.
• Prior experience in a community-based support role or social services.

• Knowledge of local resources and social services available in the Paarl area.
