Community Support Facilitator

Our client is seeking a proactive and empathetic Community Support Facilitator to join their team in Stellenbosch, Western Cape.

• Identify community needs and gaps in service provision.
• Facilitate access to social welfare programs, healthcare, education, and employment opportunities.
• Provide guidance and support to individuals and families navigating various support systems.
• Develop and maintain relationships with local service providers and community organisations in Stellenbosch.
• Organise and conduct community awareness sessions on available resources.
• Assist clients in developing personal action plans to achieve their goals.
• Monitor client progress and the effectiveness of support interventions.
• Maintain accurate and confidential client records and submit regular reports.
• Advocate for the needs of community members with service providers.
• Collaborate with team members to enhance service delivery and program impact.

• Diploma or Bachelor's degree in Social Work, Community Development, Sociology, or a related field.
• Minimum of 3 years of experience in community work, social support, or case management.
• Strong knowledge of social services and resources available in the Western Cape.
• Excellent communication, interpersonal, and problem-solving skills.
• Ability to work with diverse populations and build trust.
• Proficiency in relevant software for case management and reporting.
• Familiarity with the Stellenbosch community and its specific challenges.
• Ability to work both independently and collaboratively within a team.
• Valid driver's license and willingness to travel within the region is advantageous.
