This position is for a Registered Nurse who can administer skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Clinical Manager. Job duties include:

  • Providing services in accordance with the plan of care.
  • Making initial evaluation visits and regularly reevaluating the patient’s nursing needs.
  • Initiating the plan of care and necessary revisions.
  • Providing services requiring substantial specialized nursing skills.
  • Initiating appropriate preventive and rehabilitative nursing procedures.
  • Preparing clinical and progress notes for each patient visit and summaries of care conferences in a timely manner as per Agency policy.
  • Coordinating services.
  • Informing personnel of changes in the patient's condition and needs.
  • Counseling the patient and family/significant others in meeting nursing and related needs.
  • Participating in and presenting inservice programs.
  • Understanding and adhering to established Agency policies and procedures.
  • Processing orders and notifying physicians of patient needs and changes in condition. Completing certification/recertification orders and discharge summaries.
  • Determining the amount and type of nursing needed by each patient.
  • Referring patients requiring specialized skills to Physical Therapists, Speech Language Pathologists, Occupational Therapists, and Medical Social Workers.
  • Supervising and teaching other nursing personnel.
  • Conducting patient care conferences for assigned patients.
  • Participating in peer review and Quality Assessment and Performance Improvement as assigned.
  • Providing total patient care as needed.
  • Taking on-call duty nights, weekends, and holidays, as assigned.
  • Completing and submitting OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition within agency-defined time frames.
  • Appropriately utilizing ICD-10 codes.
