PACE Southeast Michigan is seeking a Registered Nurse Case Manager (RNCM) to coordinate care for frail elders in the PACE SEMI program. The RNCM will assess needs, plan and implement nursing care, supervise staff, and provide patient education to participants and families.
The role requires MI RN licensure (BSN preferred), a valid driver's license, and experience with home visits and a multidisciplinary team approach to improve functional capacity and autonomy for participants.
