Job Details:

• Working in the town of Sterling
• 15
  -20 patients per day
• This is a part-time position 1-2 days a week
• In addition to nurse practitioners (NP?s) candidates, physician assistants (PA?s) are welcome to apply
• Position:
  Advanced Practice Clinician (APC)
• Compensation and Benefits:
  Our compensation is a productivity-based model (Fee for Service) that offers you control to maximize your income potential. Expected compensation is estimated income range of $23,000 to $28,000 annually with no cap on productivity bonus income potential.

Responsibilities:

• Drive high-quality, safe, and cost-effective patient care in post-acute care communities
• Inpatient rounding in skilled nursing facility, inpatient rehabilitation facility, and long-term care hospital
• Collaborate with other clinician interdisciplinary teams and follow CMS guidelines
• Work closely with hospice, palliative care, and home care agencies for safe patient transitions
• Ensure timely documentation and billing through our EMR (Gehrimed) meeting Medicare guidelines

Qualifications:

• Board Certified FNP, AGACNP, ACNP, AGPCNP, or PA-C
• Willing to train experienced candidates as well
• Licensed in State
