Responsibilities include, but are not limited to:- Provides continuity of care to inmates under the supervision of licensed professionals and in compliance with behavioral health guidelines;
- Works with identified inmates to complete Medical Assisted Treatment intake packets;
- Schedules in‑reach appointments with community MAT providers;
- Participates in MAT in‑reach appointments and documents appointments appropriately;
- Collaborates closely with CDOC Community Behavioral Health, CDOC MAT Providers, Facility Case Managers, Community MAT Providers and other community agencies;
- Provides behavioral health community transition services to inmates releasing from CDOC;
- Identifies inmates that are currently participating in MAT and/or eligible for MAT services in assigned facilities;
- Provides psycho‑education such as MAT education and community resource information to the inmate population under the supervision of a licensed professional;
- Maintains a database and tracks inmates participating in the MAT in‑reach service;
- Monitors inmate treatment and documentation to assure compliance with standards, assures continuity of care and completes assigned tasks;
- Position may be required to provide coverage for other state facilities as needed.
- Applicant must possess a bachelor's degree from an accredited institution in one of the following mental health fields: counseling, social work, psychology, or marriage and family therapy. You must attach a copy of your transcripts that contain your name, name of the accredited institution, type of degree received and date degree was conferred.
Related experience in behavioral health services will substitute for the degree requirement on a year‑for‑year basis. Work experience less than full time will be prorated.
Preferred Qualifications- Experience working in a Colorado State correctional environment.
- Experience in processes such as relaxation training, social skills groups, or recreational activities.
- Experience scheduling psychiatric clinics.
- Experience providing input needed for treatment and management planning; to include observations of behaviors, problems, symptoms, and response to treatment interventions.
- Ability to provide direct behavioral health support services to clients;
- Excellent written and verbal communication skills.
