Patient Care Coordinator
We are a trusted source for home care services, driven by our belief that seniors deserve more in-home care is our ultimate responsibility. We are looking for our Care Coordinator to promote our services and to provide high-level coordination of our home care services.

Job Responsibilities:
As a Patient Care Coordinator, you will serve as a business development liaison between Northern Virginia Home Care clients, staff, other healthcare facilities, physicians, community organizations, retirement communities, assisted living, and skilled nursing facilities. You will be responsible for growth within your market, will also be expected to lead other members of the team in accomplishing this goal. You will serve as a liaison between clients, families, caregivers and direct support professionals.

You will be responsible for client intake, staff onboarding, training, scheduling, and other administrative responsibilities as necessary.

Your specific duties in this role will include:

  • Maintaining ongoing communication with case managers, discharge planners, and physician office staff; conducting frequent presentations, both formally and informally, in order to build a strong network of referrals.
  • Attending hospital case manager/discharge planner meetings/case conferences to discuss the agency’s patients and also to provide education related to any changes in home health care guidelines.
  • Compiling information needed to establish definitive patient home care plans, including assessment of the appropriateness of requested services
  • Communicating information regarding the patient to the clinical supervisor and/or primary care nurse; making agency administrative staff aware of additional patient needs and arranging for medical supplies as necessary.
  • Documenting all supporting records in accordance with regulations of applicable regulatory agencies.
  • Consistently meeting established sales call objectives and completing all required account data, call logs, and Quarterly Sales Plan forms as directed by the Sales Manager.
  • Actively participating in the agency’s business growth meetings, strategic planning meetings, company in-services, and required activities as determined by the Sales Manager.
  • Assisting the Sales Manager and Sales Trainer in the orientation and mentoring of other staff related to business growth.
  • Promoting service excellence and displaying the highest level of business professionalism at all times.
  • Maintaining ongoing communications with clients, families and caregivers in order to build a strong reputation of caring for clients as well as the caregivers.
  • Facilitate client intake process, caregiver identification, negotiations and onboarding.
  • Maintaining compliance with Virginia Department of Health (VDH), and Virginia Department of Behavioral Health and Developmental Services (DBHDS) regulations for the organization, and for the Direct Support Personnel, including progress reports, training, and other tasks as necessary.

Job Requirements:

As a Patient Care Coordinator, you must have an outgoing and engaging personality with strong presentation and marketing skills and the ability to function efficiently in a stressful work environment. It is also important that you have exceptional verbal and written communication skills and the ability to quickly build rapport and develop working relationships with referral sources, clients, families and caregivers.
Specific qualifications for the role include:

  • Minimum 1-3 years of home healthcare or related healthcare marketing experience required along with a proven track record of meeting established goals and sales quota numbers.
  • Current state license as a Registered Nurse, Licensed Practical/Vocational Nurse, Physical, Occupational or Speech Therapist, Physician’s Assistant, strongly preferred.
    Bachelor’s Degree in Business or a health-related field, preferred.
  • Must have a valid state driver’s license and appropriate auto insurance.
  • Availability for required overnight travel during training and for regional and annual meetings.
